Data Jack - Requesting a Data Jack

1. Contact the Information Technology Service Desk via phone at 920-424-3020, put in your own request at uwo.sh/service-portal, or email itservicedesk@uwosh.edu.

2. Provide the following information

   • Building

   • Room

   • Jack Number (jacks are labeled)

   • Account number to charge the Telecom work 

Costs Associated with Requests

Costs will be determined by Telecommunications.  

Install Data (jack not there)

• $257 per jack

  • $150 to install 

  • $107 to activate 

Activate Data (jack there)

• $132 per jack

  • $107 for connection to the switch

  • $25 for activation

Existing Data Jack Reactivation

• $25 per jack if jack was activated before
